Member365 includes a variety of options for creating forms: you can create polls and surveys, tests and quizzes, and forms that will be used to fill in directory listings or user/organization profiles.

One use of forms is to collect information from individuals and organizations in order to better serve them when compiling directories or creating mailing lists.
Polls and Surveys
If you’re looking to gather opinions from large groups of users, polls and surveys are a great way to do it.
Quizzes and Tests
If you’d like to offer your members ways to test their knowledge and skills — or simply offer them a fun distraction — building a quiz or test is easy. Quizzes pull randomly from a pool of questions you assign to a specific form, and can be made to show the users their results afterwards, while tests offer them a fixed selection of questions that an administrator must grade afterwards.
Other Form Types
There are a few form types not covered by these categories: web submission forms can be presented to your users in the Member Portal in case you need to collect information for a reason not covered by other forms, and email signup forms create a field that can be embedded into a website in order to allow users to sign up for email communication.
